Our Support Plan - The Outcome Star

Our outcome star is a vital tool that helps us work together to measure the positive changes in your life. Each plan is designed to suit individual service users, providing a visual representation of your current situation and the areas you wish to enhance.

Think of your day-to-day life: our outcome star allows you to pinpoint the aspects you want to improve or change. This plan serves as a motivating guide for you and your support worker, fostering a focused approach during your sessions.

As you engage with your outcome star, you'll witness tangible improvements after completing a second plan. This can significantly boost your self-esteem and strengthen your relationship with your support worker.

At SiLS, while we offer housing-related support, the direction of your supportive living services is entirely your choice. You have the autonomy to select which areas of your life you want to develop further.

Some focus areas may include:

  • Alcohol dependency
  • Family issues (especially for service users who are parents)
  • Mental health and recovery
  • Learning difficulties
  • Autism and Asperger's syndrome
  • Age-related challenges
  • Health management (for longstanding physical health issues)
  • Employment aspirations

Our visual outcome star highlights your current status, indicating which areas are satisfactory and which require improvement.

It prompts meaningful discussions regarding:

  • Your desired outcomes by the next review
  • Necessary changes to achieve these goals
  • Action steps required for these changes
  • Responsible parties for each action
  • The potential need for external support
  • Timeline for completing each step

Your assessed needs guide the development of your outcome star, but it is ultimately your priorities that shape its content. We firmly believe that it's your life and your choice, which serves as the foundation for your first outcome star. Typically, this initial supported independent living plan is completed within 3-4 weeks after starting our services, allowing ample time for your support worker to understand your needs better.

 

 

Our Action Plan

Once your outcome star is complete, we collaborate to develop an actionable plan that facilitates meaningful changes in your life. The goals you set on your outcome star adhere to the S.M.A.R.T criteria—Specific, Measurable, Attainable, Realistic, and Timely—ensuring they are geared to your success.

Reviewing the Outcome Star

Regular reviews of your outcome star are conducted in line with the goals outlined in your action plan. Typically, we revisit your plan every three months. This ongoing assessment allows you to observe the positive changes in your life, track your progress, and identify new goals to work towards, with the aim of a continual journey of improvement towards independent living.
